- Official GRE scores. Request Educational Testing Service, PO Box 6151, Princeton, NJ 08541-6000 forward your GRE score report using our institution code #4854. There is no department code. Score reports over five years old (from the test date) are no longer valid.

- The UW application fee payable to the University of Washington ($50 U.S. drawn on a U.S. bank)
- Official transcripts and degree statements
- Request Educational Testing Service, Educational Testing Service, Princeton, NJ 08541-6000, telephone 1-888-863-3544 (within the U.S. or Canada) or 1-609-771-7267 (elsewhere in the world), http://www.toefl.org, to forward your TOEFL score report using our institution code #4854. There is no department code. MLT and IELTS scores should also be sent to Graduate Admissions. English proficiency score reports over two years old (from the test date) are no longer valid.

Note: International applicants must mail a copy of the application, together with transcripts, to Graduate Admissions.
U.S. citizens, permanent residents and immigrants:
You do not need to mail any materials to Graduate Admissions if you complete the application and pay the fee on-line by credit card, unless you have earned a bachelor's or higher degree from an institution outside the United States or its territories. If so, then you must print a copy of the application and send it, together with an official copy of the transcript and degree statement, to the following address:
